Minooka CCSD 201 carries accidental medical insurance (covers medical expenses in excess of your personal/group insurance) for students injured in accidents on school premises. If you would like to purchase ADDITIONAL VOLUNTARY accidental insurance, please see the following information:

If your child has been involved in an accident during a school sponsored activity, please contact Mary Robinson in the Business Office at mrobinson@min201.org or 815-467-2588. The forms below should be completed after notifying the business office.
